Due to new insurance laws (for patients on any insurance other than Medicare) please have your primary care doctor (PCP) send us a referral and obtain an authorization number from your insurance for us prior to making an appointment.
If you are a new patient calling this office, this office will not be able to give you an appointment date right away. Once your insurance information is taken down, it will be verified through your insurance and if active, the office will call you back to make your appointment. This process may take up to 2-3 days. IF YOU HAVE CHANGED YOUR INSURANCE, PLEASE CONTACT THE OFFICE PRIOR TO COMING IN!

Insurance verification is REQUIRED prior to appointment. If you are unsure whether we are a preferred provider, call your insurance Member Services number which is usually located on the back of your card or call our office for further information.

What should you do in advance to make sure your appointment runs smoothly?
Please arrive 30 minutes earlier than your scheduled appointment to sign-in, relax and enjoy the office ambiance.

We strive to stay on schedule and value your time. New patients may require as much as 30 minutes for their initial evaluation depending on their condition.

If you have had any changes to your insurance, health or medication history, please inform the doctor and the office staff so that we may update your records. If you have moved or have a new contact number or e-mail, please let us know so that we have the most up-to-date information on you!
Should you require medical records from our office to be sent to a physician of your choosing, please contact the office. A Release of Medical Records Form will be required (and filled out in person) in order to give our office permission to send the records to your designated facility.
Please understand that a request for medical records is a lengthy process and that our office requires a minimum of forty-eight (48 hours) to process your request from the time of review of your completed form.

Completing the Patient Registration Form will save you time on your initial visit!
If you have trouble printing, or do not have access to a printer, please arrive 15-20 minutes early to complete the form.
