To make an appointment, please call our office at (352) 513-4867 or please use our Contact Us form in the Contact tab to submit a request for one! Due to new insurance laws (for patients on any insurance other than Medicare) please have your primary care doctor (PCP) send us a referral and obtain an authorization number from your insurance for us prior to making an appointment.
